- Complete the Applicant section if you qualify and notate on the application you are requesting a second permit.
NOTE: You will
NOT need to have an authorized health care specialist complete the Eligibility section.
We will already have this on file if the permit is not expired.
- You must keep a copy of the completed application on your person or in the vehicle and provide it to any traffic officer upon request. Make and keep a copy before submitting the application to WisDOT.

No fee is required to get a second DIS ID permit. However, applications made at a
local DMV customer service center that provides DIS ID permit service are subject to a
counter service fee.
