Vehicles involving trusts

Did you buy or receive a vehicle from a trust with a Wisconsin title? Complete the following:  

  1. Trustee Statement for Certificate of Title (MV2790)
    • Signed by the principal trustee(s) as seller(s)
    • The name of the trust on this form must be an exact match to the name of the trust on the Wisconsin Title. We cannot accept the trust documents from a lawyer in place of this form.
  2. ​The Certificate of Title 
    • ​Signed by the principal trustee(s) as seller(s)
    • ​​An odometer reading​ completed by the seller(s) on the back of the title for vehicles less than 20 model years old.
  3. Use this​ MV1 Online Application to pre-fill the form and calculate fees ​
