Inspector Application & Credentials

For inspector applications, applicants must include DT2001, DT2085 (Inspector # will come later), 2019 SIRT (see Attachment A in Technical Bulletin), NHI 130055 or 130056 with WI PE Certificate, all NHI certificates, and other pertinent inspection certificates. Combine all documents into one PDF document. Please submit the PDF of application and or separate inspection certificates at the following link:
