Call for Service form
The call for service form is a form used to gather information from MACH CAD. Information from calls for service generated within MACH CAD is auto-populated into the TraCS call for service form. Once the call for service CAD information is on the TraCS call for service form it can be linked to other TraCS forms created under the same call for service number. The call for service number is equal to the police number on TraCS forms. The TraCS call for service form creates a more comprehensive case within TraCS tying forms created for enforcement action to the information contained within an agencies CAD system.
Car/deer crash form
This is an abbreviate form of the MV4000e that may be used to report single unit, property damage only deer or other non-domesticated animal crashes. This form is submitted electronically to DMV.

Crash (MV4000e) form
Within the crash form contains the Amended and Fatal Supplement forms. The Crash form is the electronic version of the Wisconsin Motor Vehicle Accident Report form. Reportable crashes can be submitted to the DMV electronically. A crash data file and tiff file is transferred to the DMV FTP server. This data is then uploaded daily to the corporate database. Law enforcement is able to archive crash data on their servers and generate copies as necessary. The MV4000e contains all the fields on the paper MV4000 arranged in a different order.
The Fatal Supplement form is required whenever there is a traffic crash resulting in a fatality. The death must occur within 30 days of the crash to be considered a traffic fatality. This form replaces the paper MV3480 Fatal Supplement Form when submitted as part of a TraCS electronic crash report and is necessary to comply with the requirements of the federal Fatality Analysis Reporting System (FARS).
The Amended form is the same as the MV4000e, with the addition of a Document Number Override field. This field contains the original document number that is amended. The crash data from this form is also submitted to the DMV FTP server.
Driver condition or behavior form (MV3141)
Wisconsin law, SS 343.16 (5) (6) allows the Department of Transportation to reexamine drivers when the Department has “good cause” to believe that the driver is incompetent or otherwise not qualified to be licensed. Beginning January 1, 2018, law enforcement agencies began to use the TraCS Pursuit form to gather police pursuit data as required under Wisconsin Statute 85.07(8)(b). A “police pursuit" means an active attempt by a traffic officer in a police vehicle to apprehend one or more occupants of a moving motor vehicle, the operator of which is resisting apprehension by disregarding the officer's visual or audible signal to stop his or her vehicle, increasing the speed of the vehicle or extinguishing the lights of the vehicle. Completed Pursuit forms are then transmitted through TraCS, collected, and maintained by the Department of Transportation. Annual pursuit data for all Wisconsin law enforcement agencies is subsequently made available for traffic safety entities, law enforcement, legislators, and other interested parties.
The TraCS task form is a case management tool used to track tasks created during an investigation. The task form gives agency managers the ability to see the progression and completion of tasks by officers assigned to an investigation. These individual forms within a case can later be used to build an outline detailing who was involved in a case, what tasks they were assigned and completed, and when those tasks were done.
