WisDOT EEO Technical Assistance Guide


The FHWA-1391 is an annual workforce report required by the Federal Highway Administration (FHWA) for all prime contractors and subcontractors working on WisDOT Federal-Aid highway projects with a current prime contract or subcontract of $10,000 or more. 

  1. Is company currently working on a WisDOT highway project either as the prime or subcontractor?
    • ​​No - FHWA-1391 not required to be submitted.
    • ​​Yes - Go to next question.
  2. Does the WisDOT project have federal funding (Federal-Aid)? See FAQ #1 to determine funding.
    • No - FHWA-1391 not required to be submitted.
    • Yes - Go to next question.
  3. Is the amount of work (contract) company doing equal to or exceeds $10,000?
    • No - FHWA-1391 not required to be submitted. 
    • ​​Yes – FHWA-1391 is required. Complete the form and submit.

FHWA-1391 forms are due by August 19, 2022.

Contractors are required to report the last full week they worked in July 2022 (which could be week ending 7/2, 7/9, 7/16, 7/23 or 7/30).

Completed reports or questions should be sent by email to DOTLaborComplianceManagementSystem@dot.wi.gov; with Company Name 1391 July 2022 in the subject line.​
