For inspector applications, applicants must include
DT2001,
DT2085 (Inspector # will come later),
WisDOT DTSDIn-Service Structure Inspection Refresher 2024, NHI 130055 or 130056 with WI PE Certificate, all NHI certificates, and other pertinent inspection certificates. Combine all documents into one PDF document. Please submit the PDF of application and or separate inspection certificates at the following link:
Once submitted, we will review your information and update as necessary in HSI and our files. Inspector applicants will hear back from us in the near future.
