​​​​If ​your title, plates or renewal stickers are lost, stolen or need replacement, select one of these options to conveniently replace them online for fastest service, by mail or in person:

Before you get started:

  • If your title is held by a lien holder (lender), check title to lien holder for more information
  • You must be the owner of the vehicle to apply for a replacement title
  • Wisconsin Dealers are prohibited from using this system. Unauthorized use can lead to suspension of your license

Apply online

Have these ready:

  • Your Wisconsin Driver License or ID card number
  • The last four digits of your Social Security number
  • Your date of birth

  • OR

  • Your full name
  • Your complete Social Security number
  • Your date of birth

  • AND

  • A valid email address. You will receive notification of completion from emvpublic@dot.wi.gov. If not found in your inbox, you may want to view your spam folder.
  • The vehicle identification number (VIN) for the vehicle found on your certificate of registration or on the vehicle
  • Payment Information
    • Credit/debit card (convenience fee applies) or
    • Checking/savings account and avoid the convenience fee
You should receive a replacement title in the mail within 7-10 business days after completing the online application.

  • Print a Replacement Title Application (MV2119). This form must be signed by the current owner on record.
  • Payment information
    • Check or Money Order (do not send cash) for $20 made payable to: Registration Fee Trust

  • Driver license/identification card as proof of identity
    • If proof of identify is not provided, DMV will mail the title to the vehicle owner or lien holder
    • If someone else is picking up your replacement title, a Permission to Pick Up Title Form (MV2932​) is required with proof of identity
  • Payment information
    • $20 for your replacement title
    • There is a $5 counter service fee for in-person service

  • Personalized and some special license plates are not available for replacement using this online service.
  • Review the restrictions page to check whether this online service will work for your license plate(s).
  • Your license plates must be replaced if one or both plates have been lost, stolen, damaged or are illegible
  • You must be the owner or lessee of the vehicle to apply for a replacement license plate
  • A new license plate number, certificate of registration and stickers will be issued to you
  • Destroy the old plate(s) when you receive your new license plates

  • The vehicle identification number (VIN) for the vehicle found on your certificate of registration or on the vehicle
  • Payment Information
    • The fees vary based on plate type; the online application will calculate it for you
    • Credit/debit card (convenience fee applies) or
    • Checking/savings account and avoid the convenience fee

You should receive replacement plates with new numbers/letters in the mail within 4 weeks after completing the online application.

    • Renew license plates online and updated stickers and Certificate of Registration will be mailed to you

Have these ready:
